In patients with very severe aortic stenosis (AS), early traditional valve replacement (VR) surgery (while asymptomatic) may be associated, compared to VR performed at a chosen time following a conservative approach (onset of symptoms), with rates of operative mortality and death during follow-up (composite endpoint) which are:

A Korean randomised trial included 145 patients of this type (valve area ≤0.75 cm2; peak aortic jet velocity ≥4.5 m/sec or mean pressure gradient ≥50 mmHg) and showed that deaths were very significantly less frequent in the group who underwent early VR (1% vs. 15%). Deaths from any cause during the follow-up period were also less frequent in this group (7% vs. 21%).
