Very
severe aortic stenosis: choosing the right time to perform valve
replacement

In
patients with very severe aortic stenosis (AS), early traditional
valve replacement (VR) surgery (while asymptomatic) may be
associated, compared to VR performed at a chosen time following a
conservative approach (onset of symptoms), with rates of operative
mortality and death during follow-up (composite endpoint) which are:

Korean randomised trial included 145 patients of this type
(valve area ≤0.75 cm2;
peak aortic jet velocity ≥4.5 m/sec or mean pressure gradient
≥50 mmHg) and showed that deaths were very significantly less
frequent in the group who underwent early VR (1% vs. 15%). Deaths
from any cause during the follow-up period were also less frequent in
this group (7% vs. 21%).
